Fill out the request if you are just planning for possible future needs, or if you have an actual date or dates when you will need backup care, whether it's next week or three months from now. We'll search for backup care options in your community, tailored to your request.

• Print a copy of the Just in Time Care Invoice for Services Provided. After care takes place, complete this form, sign it, have your backup care provider sign it, and mail or fax it to us.
